The coma and recovery system comes into effect when your character is critically injured. This system is designed to add risk and consequence to combat by temporarily limiting your abilities and increasing your vulnerability. A coma is followed by a recovery period, during which penalties gradually decrease over time.

Coma & Recovery
12 hours in coma, followed by 24 hours recovery and 72 hours kill protection.
Recovery severity eases every 15 minutes as penalties step down.
During Coma (12h)
No actions; chat and messages remain available.
Accommodation can be robbed; you may lose stored items.
Hospital can be robbed; you may lose items you are wearing.
Friends online in the same city reduce robbery chances.
City spots are easier to attack, including spots guarded on behalf of others.
Respect temporarily decreases.
Street Muscle continues but becomes extremely slow due to lack of leadership.
Recovery Period (24h)
Lower success rates on actions.
Cooldowns up to 50% longer.
Incoming damage up to 35% higher.
No extra health purchases.
Damage taken in group actions (crew jobs, spot attacks) can be lethal.
Respect temporarily decreases.
Family Impact
If Don, Consigliere, and Sottocapo are all in coma simultaneously:
Family attack bonuses are disabled.
Defense bonuses are removed.
Family salaries stop.
Note: If you successfully shoot someone who is in the same family as you, that person will not go into a coma, you die instantly instead.
